2020 General and Special Elections: Absentee or Mail-in Ballot or Ballot Application Challenge Form

Complete, print, and sign this form.

Printed and signed forms must be delivered to the County Board of Election at 1400 John F. Kennedy Blvd., City Hall Room 142 by 5:00 PM on Friday, October 30, 2020, along with a certified check or money order of $10 per challenge for the challenge to be considered filed.

If a challenger is submitting multiple challenges a single certified check or money order for the aggregate amount will be accepted. Only candidates, political parties and bodies, and authorized watchers may submit challenges.
